NHM JK CHO Admit Card 2021: Written examination of eligible candidates for six (6) months Certificate Programme in Community Health for Mid Level Health Providers (MLHPs) shall be held on 24th February 2021, from 2:00 PM to 4:00 PM at various venues. The list of eligible candidates is also released by the officials for Jammu and Kashmir Division separately. The eligible candidates shall submit the attached format duly filled by the candidate and collect Admit Card in person from National Health Mission office, Regional Institute of Health & Family Welfare, Nagrota, Jammu for Jammu Division and Ground Floor, Block- A, Old Secretariat Srinagar for Kashmir Division w.e.f. 13th February 2021 to 22nd February 2021 (10:00 AM to 4:00 PM).
